Well, DS4 now has an appointment with an optometrist from covd.org. The receptionist was very reassuring, even as I shared my whole, "Really, I'm not a crazy mom!" story. She actually said, "Moms usually know" and "4 is the right age to do this, so you're right on schedule."

Whew!

So I'm feeling better. If the vision stuff comes back clean, I may chat up my friend the pediatric neuropsychologist, who knows DS4, just to see what he thinks. I'm certainly NOT looking for a diagnosis. "All's well" would be my ideal! But I just can't shake the feeling that something might be up.
