The Iowa goes pre-K-12th grade.
If you have a college degree and if your district allows it, you can administer the Iowa test to your own child (assuming you order from a company that allows this--some do not, though the test itself allows it. Confusing, I know..See below for a link to solve this problem.). You just have to agree not to deviate from the testing script.
I gave DS7 the age-correct test last year, but I'm considering giving him the next grade up this year. It was just SOOOOO painfully easy! I don't think you want to go too high up, to really stretch them, just because you don't want to have to go through remediations for your child who is operating 4 grades above age level (or whatever). The irony there would be lost on the schools, I suspect...
But if you want the test to be truly evaluative, then you may want to shoot higher. If you don't want to skip your child ahead any further than he already is, you might want to stick with the grade you want him in for re-entry purposes, just to simplify matters.
