I think whether it is an over or underestimate (and my bias is in the under direction), it is clear that we do not have a good research or societal handle on either the behavior or its prevalence.

Scaling the 6.1 in 1000 statistic I cited earlier by a factor of 5 to account for under-reporting results in a proportion of 30.5/1000 or about 3%, far lower than the 1-in-5 statistic.
