This seems really odd. I would talk to the Gifted Coordinator, if there is one, or the tester, and ask what is the procedure for requesting a 'second opinion' because the scores and the child just don't match up. You might also want to read '5 Levels of Giftedness' by Deb Ruf just to get ideas for what observations are meaningful.

The main question is how to solve your daughter's current educational misfit. What ideas are on the table at her school? The teacher observation alone tells me that something needs to be done.
