Are these scores determined by the grade level or by the child's age? How could this affect an accelerated or early entranced child?

They're normed for grade level. An accelerated or early entranced child's RIT score would be normed for his/her grade. This would usually reflect a lower percentile unless they're at or above the 99% for their grade. Does that help?
