    Does anyone know how to get a single copy of the Iowa Acceleration Scale form? The folks at great potential press only sell them in a set of ten forms, for $170.

    If this is for informal use at home, then here is a post from an old thread which helped verify that a family can get by with just a manual, without needing a set of forms. Without forms, follow the examples in chapter 4 and jot your answers on a blank sheet of paper.

    We used them a few years ago. Just Google the forms, and you will find that several schools post them as PDF's. Print them and you are set!

    The replication and posting of copyrighted materials is theft of intellectual property. Each page of the IAS form is plainly marked with the copyright year and "Do Not Copy".

    Ironically, the website of an educator engaging in this illegal behavior indicates this educator is also teaching "values" and "ethics" (while role-modeling a lack of ethics by engaging in copyright infringement). Because of these violations, some may find it difficult to trust this educator, or those who promote the illegal practice... but may find it wise to question their judgement, teachings, or advice.
