    Has anyone had their child have administered a "self perception profile" in their testing? Is this what they would test if a child had anxiety?

    There are various self perception or apperception measures, which can be used to assess social-emotional indicators, one of which is often anxiety. Some examiners routinely administer screening instruments, even when social-emotional concerns are not part of the referral question. (In one of my prior jobs, I did something like this, mainly because many of the referral questions were rather vague.)
